Photography, Video & Media Consent
This Photography, Video & Media Consent Agreement ("Agreement") is entered into between Total Taekwondo ("the Club") and the individual signing this Agreement ("Participant"), or where the Participant is under the age of 18, their Parent or Legal Guardian.
The purpose of this Agreement is to obtain consent for the creation and use of photographs, videos and other media captured during Club activities.
1. Purpose
Total Taekwondo regularly captures photographs and video footage during:
Training sessions
Gradings
Competitions
Demonstrations
Club events
Seminars
Award ceremonies
Promotional activities
These images and recordings help celebrate achievements, promote the club, educate members and support the continued growth of Total Taekwondo.
2. Consent
By signing this Agreement, I voluntarily grant Total Taekwondo permission to photograph, film and record the Participant during any official club activity.
This includes:
Still photographs
Digital photography
Video recordings
Live streaming
Audio recordings
Promotional interviews
Group photographs
Individual photographs
3. Permitted Uses
I understand these images and recordings may be used for lawful purposes including but not limited to:
Club website
Social media platforms
Printed marketing materials
Posters
Flyers
Brochures
Newsletters
Press releases
Competition reports
Club presentations
Educational material
Instructor training
Promotional videos
Advertising campaigns
Internal club communications
This permission includes future use unless consent is withdrawn in accordance with this Agreement.
4. Social Media
I understand photographs and videos may appear on official Total Taekwondo social media channels including platforms such as:
Facebook
Instagram
TikTok
YouTube
LinkedIn
Once content has been shared publicly, Total Taekwondo cannot guarantee complete removal from third-party sharing, screenshots or archived copies.
5. No Financial Compensation
I acknowledge that:
I will receive no payment or royalty.
I have no ownership rights over photographs or videos taken by Total Taekwondo.
The Club owns the copyright in media created by or on behalf of the Club unless otherwise agreed in writing.
6. Image Editing
I understand that photographs and videos may be:
Cropped
Resized
Colour corrected
Edited for quality
Combined with other media
Used alongside text or graphics
The Club agrees that images will never be edited in a way that is misleading, defamatory or inappropriate.
7. Safeguarding Commitment
Total Taekwondo is committed to safeguarding all children and vulnerable adults.
The Club agrees that:
Images will be selected responsibly.
Images intended to embarrass, exploit or humiliate participants will never be used.
Care will be taken to avoid identifying children by full name alongside photographs unless additional permission has been obtained.
Appropriate safeguarding guidance will be followed at all times.
8. Data Protection (UK GDPR)
Personal information and images will be processed in accordance with:

Total Taekwondo Privacy Policy
Photographs and videos are processed under the lawful basis of consent.
Images may be stored securely using digital storage services employed by the Club.
Only authorised club personnel will have access where appropriate.
9. Withdrawal of Consent
Consent may be withdrawn at any time by contacting Total Taekwondo in writing.
Following withdrawal:
Future use of photographs and videos will cease where reasonably practicable.
Existing printed publications cannot be recalled.
Previously distributed promotional materials may remain in circulation.
Content already shared online may not be capable of complete removal due to sharing by third parties.
Withdrawal of consent does not affect the lawfulness of processing carried out before consent was withdrawn.
10. Competitions & Public Events
I understand that photographs or video taken at competitions or public events by:
Event organisers
Spectators
Other clubs
Media organisations
are outside the control of Total Taekwondo.
11. Release of Liability
I release and discharge Total Taekwondo, its instructors, volunteers, assistants, committee members and authorised representatives from any claims relating to the authorised use of photographs or recordings made in accordance with this Agreement.
This release includes claims relating to:
Privacy
Publicity rights
Copyright
Image use
except where required by law.
12. Duration
This Agreement remains in force until the earliest of:
Written withdrawal of consent;
The Participant ceasing membership and requesting withdrawal; or
Replacement by a newer signed consent agreement.
Digital Signature
By signing electronically below, I confirm that:
I have read this Agreement in full.
I understand its contents.
I voluntarily give my consent.
I acknowledge that an electronic signature has the same legal effect as a handwritten signature.
Medical Declaration
This Medical Declaration and Health Waiver ("Agreement") is entered into between Total Taekwondo ("the Club") and the Participant, or where the Participant is under the age of 18, their Parent or Legal Guardian.
The purpose of this Agreement is to ensure the health, safety and wellbeing of all participants whilst taking part in Total Taekwondo activities.
1. Fitness to Participate
I confirm that, to the best of my knowledge, I (or my child) am physically and mentally fit to participate in martial arts training, including but not limited to:
Taekwondo classes
Sparring
Pad work
Self-defence training
Competitions
Demonstrations
Fitness exercises
Grading examinations
Club events
I understand that martial arts is a physical contact sport and carries inherent risks of injury.
2. Medical Conditions
I declare that all known medical conditions, illnesses, disabilities, allergies and injuries have been disclosed to Total Taekwondo before participation.
This includes, but is not limited to:
Asthma
Epilepsy
Diabetes
Heart conditions
High blood pressure
Joint disorders
Previous fractures
Back or neck injuries
Concussion history
Allergies
Autism Spectrum Disorder (ASD)
ADHD
Anxiety or other mental health conditions that may affect participation
Learning difficulties
Medication requirements
Any condition that may affect safe participation
3. Ongoing Duty to Inform
I understand that it is my responsibility (or the responsibility of the Parent/Guardian) to notify Total Taekwondo immediately if:
A new medical condition develops.
Medication changes.
An injury occurs.
Surgery is undertaken.
A doctor advises restrictions on physical activity.
Any other circumstance may affect safe participation.
I acknowledge that failure to disclose relevant medical information may place myself, my child and others at unnecessary risk.
4. Medication
If medication may be required during training, I agree to notify the Club in advance and provide appropriate instructions where necessary.
I understand that instructors are not medical professionals and cannot administer medication unless legally permitted and previously agreed.
Participants remain responsible for bringing any required medication (such as inhalers or emergency medication) to every class unless alternative arrangements have been agreed.
5. Emergency Medical Treatment
In the event of an accident, illness or medical emergency, I authorise Total Taekwondo to:
Contact emergency services (999) where considered necessary.
Contact my nominated emergency contact.
Provide basic first aid within the limits of the instructor's training.
Share relevant medical information with healthcare professionals where necessary to protect my health or safety.
I understand that every reasonable effort will be made to contact my emergency contact before further treatment where circumstances allow.
6. Medical Advice
I acknowledge that:
Total Taekwondo instructors are not doctors or healthcare professionals.
Any advice given relates only to training participation.
Medical advice should always be sought from an appropriately qualified healthcare professional.
If I have any doubts regarding my fitness to participate, I will seek medical advice before attending training.
7. Risk Acknowledgement
I understand that participation in martial arts involves inherent risks including, but not limited to:
Bruising
Sprains
Muscle strains
Cuts
Broken bones
Joint injuries
Concussion
Accidental contact injuries
While Total Taekwondo takes reasonable precautions to provide a safe training environment, it is not possible to eliminate all risks associated with martial arts training.
8. Limitation of Liability
To the fullest extent permitted by law, I acknowledge that participation is undertaken voluntarily and at my own risk.
Nothing within this Agreement excludes or limits liability for:
Death or personal injury caused by negligence.
Fraud or fraudulent misrepresentation.
Any liability that cannot legally be excluded under the laws of England and Wales.
9. Data Protection
Medical information is regarded as special category personal data and will be handled confidentially.
The information provided will be used only for:
Participant safety
Emergency response
Safeguarding
Reasonable adjustments during training
Insurance requirements where applicable
Compliance with legal obligations
Medical information will only be shared where necessary with authorised instructors, emergency services or healthcare professionals.
Information will be stored securely in accordance with the UK GDPR and the Data Protection Act 2018.
Student & Parent Code of Conduct
By becoming a member of Total Taekwondo, all students and parents/guardians agree to uphold the values of respect, courtesy, integrity, perseverance, self-control and indomitable spirit. This Code of Conduct is designed to create a safe, welcoming and supportive environment for everyone.
Failure to comply with this Code of Conduct may result in disciplinary action, including suspension or termination of membership.
Student Code of Conduct
1. Respect
All students will:
Treat instructors, assistant instructors, volunteers, officials and fellow students with courtesy and respect.
Follow all instructions given by instructors promptly and without argument.
Show good sportsmanship at all times.
Respect the training venue and all equipment.
Respect competitors, regardless of the outcome of a match or event.
2. Behaviour
Students must:
Behave responsibly at all times.
Listen when instructors are speaking.
Refrain from disruptive behaviour during classes.
Avoid inappropriate language, gestures or behaviour.
Never engage in bullying, intimidation, discrimination or harassment.
3. Safety
Students agree to:
Train safely and responsibly.
Wear the correct protective equipment when instructed.
Inform an instructor immediately if injured or feeling unwell.
Never deliberately use excessive force.
Never attempt techniques beyond their level without instructor permission.
4. Attendance
Students should:
Arrive on time for classes.
Inform the instructor if arriving late.
Attend regularly where possible.
Notify the Club if unable to attend grading or pre-booked events.
5. Uniform & Equipment
Students must:
Wear a clean and presentable dobok (uniform).
Maintain good personal hygiene.
Keep fingernails and toenails short.
Remove jewellery before training unless approved for medical reasons.
Ensure sparring equipment is clean and in good condition.
6. Mobile Phones & Recording
Students should:
Keep mobile phones on silent during training.
Not use phones during class unless authorised by an instructor.
Never record or photograph others without permission from the Club.
7. Respect for Club Reputation
Students agree not to:
Bring the Club into disrepute.
Make abusive or defamatory comments about instructors, students or the Club online or in person.
Misrepresent their grade, qualifications or association with Total Taekwondo.
Parent / Guardian Code of Conduct
Parents and guardians play an important role in creating a positive training environment.
By enrolling a child, parents agree to the following.
1. Respect for Instructors
Parents will:
Treat instructors and volunteers respectfully.
Allow instructors to teach without interruption.
Raise concerns privately and respectfully rather than during classes.
2. Positive Support
Parents agree to:
Encourage effort rather than focusing solely on results.
Support all students equally.
Promote respect, discipline and good sportsmanship.
Celebrate progress regardless of ability.
3. Behaviour During Classes
Parents must:
Follow venue rules.
Avoid distracting students during lessons.
Supervise siblings attending classes.
Keep noise to a minimum while observing.
4. Communication
Parents agree to:
Keep emergency contact details up to date.
Notify the Club of changes to medical information.
Inform instructors of any concerns affecting their child's participation.
Read Club communications and respond where required.
5. Photography & Social Media
Parents agree to:
Respect the privacy of other members.
Not post photographs or videos of other children without permission.
Use social media responsibly.
Avoid posting comments that could damage the reputation of the Club or its members.
6. Competitions & Events
Parents will:
Support all competitors respectfully.
Show courtesy to referees, officials and volunteers.
Accept competition decisions with good sportsmanship.
Never coach from the sidelines unless authorised.
7. Safeguarding
Parents agree to:
Report any safeguarding concerns immediately.
Follow safeguarding procedures.
Respect the confidentiality of all members.
Anti-Bullying Policy
Total Taekwondo has a zero-tolerance approach to:
Bullying
Harassment
Intimidation
Discrimination
Racism
Sexism
Homophobia
Transphobia
Religious discrimination
Online abuse
Physical or verbal aggression
All concerns will be investigated appropriately.
Disciplinary Procedure
Where this Code of Conduct is breached, Total Taekwondo may take one or more of the following actions:
Verbal reminder.
Written warning.
Temporary suspension from classes.
Removal from competitions or events.
Permanent termination of membership.
Serious misconduct may result in immediate suspension or termination without prior warning.
Agreement
I acknowledge that I have read and understood the Total Taekwondo Student & Parent Code of Conduct.
I agree to comply with these expectations and understand that failure to do so may result in disciplinary action.
Membership Agreement & Terms and Conditions
This Membership Agreement ("Agreement") is entered into between Total Taekwondo ("the Club") and the Member, or where the Member is under the age of 18, their Parent or Legal Guardian.
By signing this Agreement, you agree to comply with these Terms and Conditions and all associated Club policies.
1. Membership
Membership provides access to Total Taekwondo classes in accordance with the membership package selected.
Membership is personal to the named member and may not be transferred or shared.
Membership begins once registration has been completed, payment has been authorised, and any required documentation has been submitted.
2. Membership Packages
Membership options are published on the Total Taekwondo website and may be amended from time to time.
Membership includes only the number of weekly classes associated with the selected package.
Family discounts may be available where applicable.
3. Payment
I understand and agree that:
Membership fees are collected by recurring card payment on a monthly basis.
Fees are calculated over a 44-week training year and spread equally across 12 monthly payments.
Monthly payments continue during school holidays and other scheduled Club closures included within the annual training calendar.
Membership fees secure my place within the Club and are not based solely on attendance.
4. Annual Registration & Insurance
All members must maintain a valid annual registration and insurance.
Annual membership and insurance fees are payable separately from monthly tuition fees.
Where payment has not been received within two weeks of becoming a member or renewal becoming due, Total Taekwondo reserves the right to collect the outstanding annual fee using the authorised recurring card payment.
Competitors may require additional membership with the relevant governing body before participating in sanctioned competitions.
5. Missed Classes
If I miss a class due to illness, holidays or other personal commitments:
Membership fees remain payable.
I may attend a suitable alternative class or location where availability permits and with instructor approval.
Make-up classes cannot be exchanged for refunds or credits.
6. Additional Events
Monthly membership includes regularly scheduled classes only.
The following may incur separate charges:
Gradings
Seminars
Training camps
Competitions
Special events
Workshops
Merchandise
Equipment
Participation in optional events is entirely voluntary.
7. Failed Payments
If a recurring card payment fails:
A £5 administration fee will be added to the next payment.
Outstanding balances remain payable.
Total Taekwondo reserves the right to suspend training until payments are brought up to date.
Repeated failed payments may result in cancellation of membership.
8. Membership Changes
Members may request to:
Upgrade membership.
Downgrade membership.
Change training days.
Update payment details.
Requests should be made in writing and will normally take effect from the next billing period where reasonably practicable.
9. Cancellation
Membership may be cancelled by providing one calendar month's written notice.
Notice must be submitted by email or in writing to Total Taekwondo.
Membership fees remain payable throughout the notice period.
No refunds will be issued for unused membership after cancellation unless required by law.
10. Refund Policy
Total Taekwondo does not provide refunds for:
Missed classes
Holidays
Illness
Non-attendance
Voluntary withdrawal during a paid membership period
Optional events already attended
Refunds may be considered only where required by law or at the sole discretion of the Club.
11. Club Closure
Where classes cannot take place due to circumstances beyond the Club's reasonable control (including severe weather, venue closure, utility failure or other unforeseen events), Total Taekwondo will endeavour to provide alternative sessions where practical.
Such closures do not automatically entitle members to refunds.
